The Community Residential Program provides opportunities for adults to live in a supportive environment as fully participating members of the community. The homes are established to facilitate individual choice, foster greater independence and community involvement, and provide a comfortable, life-enriching home life.
The community homes enable small groups of individuals to reside together, sharing their homes and building relationships with each other and staff. Individuals are provided with 24-hour a day support by trained professionals who assist in developing a person centered program designed on each person’s preferences, wants and needs.

Using a person centered approach; each person in our program participates in the development of his or her own personal plan for services and receives the guidance and support of our qualified staff. Those employing our services are encouraged and supported to live an active and fulfilling life, exploring new and interesting activities of their choice.
Individualized plans are based upon the needs, wants, and aspirations unique to each person. People are surrounded by a support team that includes the direct service staff, team leaders, administrative staff and community medical professionals who coordinates all services the person may need. Our program offers a twenty-four hour on-call system to ensure that people served, employees and families can be supported when the need arises.
